Campus Federal Image & Testimonial Release

I irrevocably consent to and authorize the use and reproduction by Campus Federal (“Credit Union”), or anyone authorized by Credit Union, of all photographs of me and or my property, and my spoken or written statements, reviews and testimonials, for all purposes, in any form, even in conjunction with my name, fictitious name or company name. 

This authorization includes the use of photographs, pictures, portraits, images and statements, reviews, and testimonials, in any and all forms and media and in all manners, including composite images or distorted representations, summations or partial quotes, for the purposes of publicity, illustration, advertising and publishing (including publishing in electronic form or on internet websites), for any product or services or other lawful uses, as may be determined by Credit Union, or anyone authorized by Credit Union. 

I release and discharge Credit Union, and those acting under the authority of Credit Union, from any and all liabilities or claims arising from or related to the blurring, distortion, use in composite form, or alteration of the image or partial quotation of written or verbal statements. 

I am providing this consent voluntarily, without expectation of compensation or reward. I waive any and all rights to review or approve the finished images, advertising copy, or finished product containing the images and statements. I also release and discharge Credit Union, and those acting under the authority of Credit Union, from any and all claims in connection with the use of the images or statements, including without limitation, any and all claims for invasion of privacy and libel. I affirm that I am over the age of majority and have the right to contract in my own name. I have read and fully understand the terms of this release.
